I noticed this as well. Switch to 16bit and you get heavy grey fog. The deal with this is, by setting it to 16bit, your FPS goes up a bit. Maybe thats what the fog is there for?

Uninstall Spearhead. Reinstall Spearhead. Install the Patch.

that fixed all my problems. runs good now.

The NO CD Patch is not a cracked version of the game. Its just the moh_spearhead.exe set to run without the CD. makes it easier to load the game. I'm lookin out for it myself.

if this is happening when you play Spearhead, then i can relate to this problem. check the console after it errors out. If it says "Can't find player spawn", then that means there are no spawn points...
