A little birdie told me it was mainly spawn points and new entities with different values which may cause problems.

If i was starting a map now (which i am ) i wouldnt place any entities into it...

I just renamed a DOH sound file i have and pak3 it in this structure.
DOH.WAV
FIRE folder
WEAPONS folder
SOUND folder
User-MOD-KDJA-Homer.pk3

And it worked fine.
