Right Mouse = Forward
Left Mouse = Fire
Middle Mouse = Alt, Wheel = Cycle Weapons
M = Right Strafe
N = Left Strafe
B = Crouch
J = Lean Left
K = Lean Right
Right ALT = Use
Space = Jump

This works the best for me. All my fingers are on or near an important key at all times.
