sometimes a guy is following me multiplayer and i stop and wait for him to walk by me and i walk up behind him and then ... MISS!

I walk up right behind them i think being too close is the problem, i think this could be because the game lags when people are close to each other: i noticed my ping jumps up uncontrollably when there is very close range firefight.
