Reformat? yea yea I know last resort but it looks like you have tried it all.

But to try and help, I would clean out all the old drivers first and uninstall the game. I would use det destroyer to totally clean out the vid drivers. Then reinstall the drivers, sound, vid, 4in1's, directx and then install the game.
