As for enemy balance, it is following a rank structure where higher ranking soldiers will be a bit tougher. Not in terms of hit points mind you (no you won't have the 5000 mega Nazi Gestapo guy

. More in terms of weapons they will use, and their accuracy, tenacity, aggression, things on that nature. Finding them more in squads than not sort of thing. His tendancy to be smart and stay / fire from behind cover rather than exposing himself to fire (making him more difficult to kill), his grenade perception and ability to retreat / throw them back at you. It's an overall combination of all these factors.
