I'll go in with you guys provided everyone is checking that the IP address is not dynamic!
There is sod all point in adding 56k'ers to this list for example. Also the lease times that cable/DSL providers have varies enormously. Eg - here in the UK some DSL ISPs will change your IP address each time your equipment is power-cycled. Given that the incumbent teleco here (BT) prefers that home users have a USB modem you can guess how many times that happens! Things are slowly changing and more people are going for routers so it's permanently connected but this still causes me problems.
Typically I'll remove IP's from the banned list after 24 hours where I know that the ISP hands out dynamic IPs. I will leave the name in there though. For the cablecos here it's not such a problem and I will typically remove the IP address after 5-6 weeks.
So how do the rest of you handle this (or have you not really thought about it)?
