It basically changes the focus of the game from "how well can you keep the gun on an opponent to hit him enough to kill him" to "how quick can you get the drop on an opponent"
In most realism mods, most weapons are one or two shot kill weapons, except of course the shotgun and rocket, which have been downgraded so much that they might as well not exist.
My favorite realsm mod is FK (or MOB clan) realism. They left the shotty and rocket alone, and upped the other weapons to be comparable. Their weapons are also equal for axis and allies, ie - smg = smg, mg=mg. rifles are the same, etc.
It makes the game faster and requires a player to be more stealthy in how he approaches a target. I still prefer base (non-realism), but realism is a must if you like new maps and it is a nice change of pace.
