the biggest factor would be streanth of schedule. If you play the good teams it puts you near the top, if you schedule pansy teams you are near the bottom where you belong. As far as teams being good year in and out that is true. There are about a dozen teams that always have good teams but there are reasons for that. When you prove yourself to be one of the best teams year in and year out you get on national TV more. That in turn helps recruting, the talented kids want to be on TV each week because that will help them get into the NFL so the best teams always get the best recruits which in turn makes them one of the best teams again

Everyone has atleast 1-3 tough games that will prove just how good they are. If that team manages to go undefeated up until they face a good team, they will still probally ranked high. An example of what you are talking about is Rutgers. They had a shitty schedule all year and are currently undefeated but tonight they will be facing another undefeated team.
