No kidding. It has fuck all to do with first or third world country status - and everything to do with human error.

Pretty fucked up error when you're refusing to help a dieing patient. I can understand clerical errors and medication/dosage mistakes, surgeical/anesthetic errors - but refusing to help someone thats bleeding to death on the floor is plain fucked-up....Of course its not confined to the americas - But I wouldnt expect something like that to happen in the richest country in the world.
