Most of the time, i use custom brushes..
Like Angry Blue..
Thats a good one if you want blood, splatter and stuff.

Photoshop, I make a new layer and erase it. Then for blood I use a blur to make it a little smoother.

there is a plugin for photoshop called xenofex ..it has a stain filter ..you can change the collor/depth of the stain and move it around
also eye candy 4000 has a waterdrops filter and you can make some great blood drops with it
