I am the Maker of the Malta custom map for Spearhead. It wont let you use user infront of the custom maps. Maybe its because Malta really isnt custom but buy naming it mp_malta_dm it puts it right into place for the game to recognize. My clan runs a dedicated server and the map is in its rotation and works fine (other then most people dont have it and get booted). Even admins have no prob. changing to it thru rcon. Like I said could just be the way spearhead is or it could be just that one map.
