The hearts a hell artifact, more or less their version of the soul cube. Once you beat the first boss you gain hell time which lets you slow everything down while you can move around at normal speed. Its charged by human souls, just find a dead marine and stand next to him with the artifact in your hands.
Yes you're a combat engineer in the uac in this version, not the same marine from doom 3.
