this was covered recently
search for "mouse" and "r3mix" as the author and you will get the thread.

solution AFAIK is to download the latest config program for the mouse.... install it so all the drivers become avaialble.... uninstall it (and the old version if its there) as well as the drivers for your mouse.... reboot.... feed the updated drivers to the mouse and reintall the MS program.

anyways i pissed about for ages doing that in different orders and something finally worked. the MS program doesnt change but depending on what driver your mouse is using some of the options in it will be limited.

the updated MS program will have an option to set advanced settings just for MoH. you assign a couple of keys like backspace and delete to buttons 4 & 5 (scroll button should already be working as #3) and then in the game you can set these bksp/del as the functions you want. if its all working good you should be able to click you button and bksp/del appears as the chosen key.

Ok..

Install the software of intellimouse optical...go to config where u can bind the buttons to functions....so mouse button 1 is fire, 2 is sec. fire, 3 is wheelmouse scroll..4 is gonna be lean left 5 is lean right...now give lean 4 the function of BACKSPACE and 5 is Delete...

Now start mohaa..go to options..and select lean left as backspace and lean right is Delete...

Eh Voila..u just binded lean to ur mouse...i must say it works pretty good...i will never go back to 3 button mouse.. if u need help email me at technoor@ny.com or icq me...152482486
